<p class="wp-block-paragraph">Our friend texted our group chat, along with a photo of her swollen belly.</p>



<p class="wp-block-paragraph">She wasn&#8217;t pregnant; she&#8217;s 51. And this wasn&#8217;t a food baby either.&nbsp;</p>



<p class="wp-block-paragraph">For weeks, she&#8217;d been having trouble emptying her bladder completely, but like most of us, she kept pushing through. It wasn&#8217;t until she realized it had been almost 18 hours since she&#8217;d peed that she finally got to the doctor.</p>



<p class="wp-block-paragraph">Turns out she had a fibroid the size of a grapefruit pressing down on her urinary tract.</p>



<p class="wp-block-paragraph">Flash forward to weeks of catheter bags while she waited to get scheduled for a full hysterectomy. Have we mentioned our health care system is overworked?</p>



<p class="wp-block-paragraph">Her experience got us thinking about how most women don’t know what fibroids are until they become a problem. So, here&#8217;s what you need to know:</p>



<h2 class="wp-block-heading"><strong>What’s a fibroid?</strong></h2>



<p class="wp-block-paragraph">Uterine fibroids are muscle tumors in your uterus &#8211; but don&#8217;t panic. While ‘tumor’ sounds terrifying, fibroids are not cancerous.&nbsp;</p>



<p class="wp-block-paragraph">You can have a single fibroid or multiple. And fibroids can be microscopic to cantaloupe-sized. And as my friend discovered, when they grow large enough, they can press on nearby organs like the bladder, creating serious complications.</p>



<p class="wp-block-paragraph">They&#8217;re pretty common; up to 80% of black women and 70% of white women will have one by age 50. Their impact can range from having no idea they exist to wreaking havoc on your well-being and quality of life.</p>



<p class="wp-block-paragraph"><strong>Why do fibroids happen?</strong></p>



<p class="wp-block-paragraph">We don&#8217;t know. Like many female-specific health issues, we need more research. Fibroid-related symptoms are often dismissed as ‘part of being a woman’. And it can take years to get diagnosed. This needs to change.</p>



<p class="wp-block-paragraph">Fortunately, change is on the horizon thanks to influential women advocating and supporting legislative initiatives for fibroid research funding. We applaud actor Lupita Nyong&#8217;o for sharing her story of silently suffering before having 30 fibroids removed. Her voice is paving the way for better understanding and care.</p>

<h2 class="wp-block-heading"><strong>What are the symptoms?</strong></h2>



<p class="wp-block-paragraph">Small fibroids typically cause no symptoms and just need monitoring. Larger fibroids can cause:</p>



<ul class="wp-block-list">
<li>Heavy / painful periods</li>



<li>Bleeding between periods</li>



<li>Bloating or fullness in lower belly</li>



<li>Frequent urination or difficulty emptying bladder completely</li>



<li>Pain with sex</li>



<li>Lower back pain</li>



<li>Constipation or rectal pressure</li>



<li>Ongoing vaginal discharge</li>



<li>Abdominal swelling</li>
</ul>



<h2 class="wp-block-heading"><strong>How are fibroids treated?</strong></h2>



<p class="wp-block-paragraph">Treatment becomes necessary when fibroids cause heavy bleeding leading to anemia, significant pain, or health complications like bladder issues. The type of treatment will depend on a variety of things including symptom severity, life stage, and personal preference. Fibroids can be treated with:</p>



<ul class="wp-block-list">
<li><strong>Medications:</strong> like pain relievers (ibuprofen, acetaminophen), birth control to reduce heavy bleeding, or hormone medications to temporarily shrink fibroids</li>



<li><strong>Surgeries</strong><strong>:</strong>
<ul class="wp-block-list">
<li>Myomectomy &#8211; removes fibroids while keeping uterus</li>



<li>Hysterectomy &#8211; removes entire uterus (what our friend had)</li>



<li>Embolization or Ablation &#8211; different procedures that reduce blood flow to the fibroids.</li>



<li><span style="text-decoration: underline;">Note</span>: with the exception of a hysterotomy there&#8217;s a high reoccurrence rate for these procedures, meaning the fibroids can grow back.</li>
</ul>
</li>
</ul>



<h2 class="wp-block-heading"><strong>When to see a doctor</strong><strong>?</strong></h2>



<p class="wp-block-paragraph">Contact your doctor if you experience:</p>



<ul class="wp-block-list">
<li>Persistent pelvic pain</li>



<li>Heavy periods that interfere with daily activities</li>



<li>Bleeding between periods</li>



<li>Difficulty emptying your bladder</li>



<li>Pressure or fullness in your abdomen</li>



<li>Painful intercourse</li>



<li>Constant fatigue or weakness (possible anemia)</li>
</ul>



<h2 class="wp-block-heading"><strong>Seek immediate care for:</strong></h2>



<ul class="wp-block-list">
<li>Severe vaginal bleeding</li>



<li>Sudden, sharp pelvic pain</li>



<li>You haven&#8217;t peed in 18 hours</li>
</ul>

<h2 class="wp-block-heading"><strong>How much period blood do you lose each month?</strong></h2>



<p class="wp-block-paragraph">I know it feels like we&#8217;re hemorrhaging every month, the reality is that for most, a typical period involves just 30-40 ml of actual blood loss over the entire cycle. That&#8217;s about 2-3 tablespoons.</p>



<p class="wp-block-paragraph">When bleeding exceeds 80 ml per cycle, doctors classify it as heavy menstrual bleeding.</p>



<p class="wp-block-paragraph">But forget the measurements for a moment. If you&#8217;re experiencing:</p>



<ul class="wp-block-list">
<li><strong>Hourly protection changes</strong>&#8211; soaking through pads or tampons every hour</li>



<li><strong>Emergency layering</strong> &#8211; doubling up on protection just to get through the day</li>



<li><strong>Marathon periods</strong> &#8211; bleeding for more than seven days straight</li>



<li><strong>Quarter-sized clots</strong> &#8211; passing blood clots larger than a quarter</li>



<li><strong>Fatigue</strong> &#8211; feeling constantly tired and weak (heavy menstrual bleeding can cause iron deficiency)</li>
</ul>



<p class="wp-block-paragraph">…it’s not just annoying. This is considered heavy bleeding and there&#8217;s usually a reason behind it.</p>



<p class="wp-block-paragraph"></p>



<p class="wp-block-paragraph">Yes, it could be because of our perimenopause hormonal fluctuations, but it could also be:</p>



<ul class="wp-block-list">
<li><strong>Endometriosis </strong>&#8211; tissue that should line your uterus decides to grow elsewhere, causing heavier, more painful periods</li>



<li><strong>Uterine fibroids</strong> &#8211; benign tumors in the muscle of the uterus that can cause a variety of symptoms like heavy bleeding and irregular periods</li>



<li><strong>Cervical or uterine polyps</strong> &#8211; small benign growths that can make periods heavier and more irregular</li>



<li><strong>Infections</strong> &#8211; can disrupt your normal cycle</li>



<li>And in rare cases, cancer</li>
</ul>



<p class="wp-block-paragraph">The good news? Depending on the cause of your heavy bleeding, there are treatments available, like:</p>



<ul class="wp-block-list">
<li><strong>Hormone-releasing IUD (Mirena)</strong> &#8211; the gold standard, reduces bleeding by 70-80% and many women&#8217;s periods stop completely</li>



<li><strong>Progestogens</strong> &#8211; help regulate irregular, heavy cycles when taken part of each month</li>



<li><strong>Birth control pills </strong>&#8211; can cut bleeding by 50% while also helping with cramps and hot flashes</li>



<li><strong>Tranexamic acid </strong>&#8211; helps the blood clot; take it only during your period to reduce bleeding by up to 50%</li>



<li><strong>MHT</strong> &#8211; less effective treatment for heavy bleeding but worth discussing</li>



<li><strong>Surgery</strong> &#8211; last resort when medical treatments aren’t working</li>
</ul>



<p class="wp-block-paragraph">Your period shouldn&#8217;t rule your life. If it does, it&#8217;s time to fight back. <h2 class="wp-block-heading"><strong>01. No one is coming</strong></h2>



<p class="wp-block-paragraph">No one is coming to explain perimenopause to you or help you sort out your symptoms.</p>



<p class="wp-block-paragraph">To get the care and support you deserve, you&#8217;ll need to get educated about your body and research the evidence-based options available to support your journey and healthspan.</p>

<h2 class="wp-block-heading"><strong>02. Vaginal estrogen is safe</strong></h2>



<p class="wp-block-paragraph">The majority of women will experience symptoms of <a href="https://thisisperimenopause.com/symptoms/genitourinary-syndrome-of-menopause-what-you-need-to-know/"><mark style="background-color:rgba(0, 0, 0, 0)" class="has-inline-color has-vivid-purple-color">genitourinary syndrome of menopause (GSM)</mark></a>: leaking urine, burning, itching, pain, and recurring UTIs are just the tip of this mo-fo of an iceberg. And these symptoms will not resolve on their own.</p>



<p class="wp-block-paragraph">Vaginal estrogen is the gold-standard treatment. And it has a boxed warning (formerly called a black box warning) that overstates the risks, causing many to forgo the benefits.</p>



<p class="wp-block-paragraph"><a href="https://youtu.be/AXxadHDUWgM?si=0ye3CN4Af71TEejO" target="_blank" rel="noreferrer noopener"><mark style="background-color:rgba(0, 0, 0, 0)" class="has-inline-color has-vivid-purple-color">Expert consensus</mark></a><mark style="background-color:rgba(0, 0, 0, 0)" class="has-inline-color has-vivid-purple-color"> </mark>is that the warning should be removed. Until that happens, don&#8217;t let the warning scare you. Vaginal estrogen will do wonders for your pelvic floor.</p>



<h2 class="wp-block-heading"><strong>03. Wake after sleep onset (WASO) is a thing</strong></h2>



<p class="wp-block-paragraph">Many women start waking up at 2 am and can&#8217;t get back to sleep in their late 30s and early 40s. Their periods are normal. They aren&#8217;t experiencing hot flashes. And they don&#8217;t have new stress that would explain this sleep disturbance.</p>



<p class="wp-block-paragraph">The world is now waking up (pun intended) to the fact that <a href="https://womenlivingbetter.org/sleep-disruption-in-perimenopause/" target="_blank" rel="noreferrer noopener"><mark style="background-color:rgba(0, 0, 0, 0)" class="has-inline-color has-vivid-purple-color">WASO is a symptom of perimenopause</mark></a>. This is important, because without this information you&#8217;ll likely start blaming yourself.&nbsp;</p>



<p class="wp-block-paragraph">This info will also help you <a href="https://thisisperimenopause.com/podcast/sleep-in-perimenopause/"><mark style="background-color:rgba(0, 0, 0, 0)" class="has-inline-color has-vivid-purple-color">find the solutions you need to start getting a full-night of sleep</mark></a> again.</p>

<p class="wp-block-paragraph">And what if those heavy periods and severe cramps aren&#8217;t normal, but actually signal something serious? For 1 in 10 women with endometriosis, this is reality. And shockingly, it can take an average of 7-10 years to get diagnosed.</p>



<p class="wp-block-paragraph">Let that sink in for a moment.</p>



<p class="wp-block-paragraph">A decade of suffering before someone finally says, &#8220;This isn&#8217;t normal, and you&#8217;re not imagining things.&#8221;</p>



<p class="wp-block-paragraph">And when we say pain, we mean debilitating, life-stopping pain. We have a friend who spent 6 years curled up on her bathroom floor in agony every month during her period. Sex often ended in tears because of the pain. To say this is not normal, is the understatement of the year!</p>



<h2 class="wp-block-heading"><strong>So what is Endometriosis?</strong></h2>



<p class="wp-block-paragraph">Due to a lack of research and funding, endometriosis is not well understood. Endometriosis occurs when abnormal tissue similar to the uterine lining grows outside the uterus as well as on, and sometimes into, other organs like the bladder, bowels, or ovaries.</p>



<p class="wp-block-paragraph">What makes endometriosis particularly tricky is that it can affect your entire pelvic region and cause pain outside of your period. It can also cause infertility.</p>



<h2 class="wp-block-heading"><strong>Here are common symptoms to watch for:</strong></h2>



<ul class="wp-block-list">
<li><strong>Severe menstrual cramps</strong> that interfere with daily activities</li>



<li><strong>Abdominal and back pain</strong> during or in between periods</li>



<li><strong>Pain during or after intercourse</strong></li>



<li><strong>Pain with bowel movements or urination</strong>, especially during your period</li>



<li><strong>Heavy bleeding</strong> during periods</li>



<li><strong>Gastrointestinal symptoms</strong> (nausea, bloating, diarrhea) that cycle with your period</li>
</ul>



<h2 class="wp-block-heading"><strong>Why is it so hard to diagnose?</strong></h2>



<p class="wp-block-paragraph">Unfortunately, endometriosis is invisible to most tests &#8211; undetectable via blood work and tricky to find with imaging. And while providers are able to make a clinical diagnosis, many still consider surgery the only definitive way to diagnose and treat endometriosis.</p>



<p class="wp-block-paragraph">Often when doctors can&#8217;t see or feel anything during routine exams, symptoms get dismissed as &#8220;normal&#8221; period pain. Combined with the ongoing issue of women&#8217;s pain being downplayed, many suffer for years before getting answers.</p>

<h2 class="wp-block-heading">Sometimes the happiest smiles hide the deepest struggles.</h2>



<p class="wp-block-paragraph">As flowers bloomed this March, you’d expect my spirits to rise. Yet, I was fighting a silent, invisible battle. Outwardly, everything looked fine—I managed all my family duties flawlessly. Inwardly, however, I felt my inner spark extinguish.</p>



<p class="wp-block-paragraph">Despite my usual drive, I struggled to maintain motivation. Normally, the quintessential get-shit-done kind of girl, I was embarrassed as I watched Mikelle cover for my lapses. I was haunted by a growing disappointment in myself and a persistent fear of not being good enough.</p>



<p class="wp-block-paragraph">If someone had told me I was depressed, I would have scoffed at them. A good friend of mine lost her life to depression, I know what it looks like. And it did not look like me. I got out of bed every day. I showered. I kept my family flourishing. My house was tidy and my dog got walked.</p>



<p class="wp-block-paragraph">Yet, despite appearing functional, I was far from it. A recent podcast episode by&nbsp;<a href="https://www.melrobbins.com/podcasts/episode-169?_kx=fPH2OXAt7tc3Y6_Quc4e8A.XHRnps" target="_blank" rel="noreferrer noopener">Mel Robbins on High Functioning Depression</a>&nbsp;opened my eyes—it&#8217;s a real, though often unrecognized condition and not officially in the diagnostic manual. It manifests when someone manages daily life outwardly but battles complete joylessness within. Think of it as treading water: from the shore &#8211; it looks like you’re swimming, but really, you’re just trying to keep your head above water.</p>



<p class="wp-block-paragraph">I’m sharing this with you because high-functioning depression often goes unnoticed. &nbsp;Too many of us tough it out alone, thinking this is just how life has to be.&nbsp;&nbsp;But that’s not the case.&nbsp;</p>



<p class="wp-block-paragraph">I’m learning that it’s okay to not have your stuff together all the time. And it’s okay to ask for help.&nbsp;</p>

<p class="wp-block-paragraph">We know this unsettling feeling. Boy do we ever! We’ve been there, done that, and have the T-shirt to prove it. It&#8217;s actually the reason we started This Is Perimenopause.&nbsp;</p>



<p class="wp-block-paragraph">From our own early days, feeling out of sorts—struggling with sleep disruptions, mood swings, and an unexplained anxiety. To the resulting helplessness, disorientation, and even self-loathing and fear &#8211; the memories are etched deep in our minds.</p>



<p class="wp-block-paragraph">And we know many of you have whispered the same words, “I’m not feeling like myself.”&nbsp;</p>



<p class="wp-block-paragraph">In fact, so many of us have been quietly voicing these concerns that the whispers grew loud enough to inspire the&nbsp;<a href="https://pubmed.ncbi.nlm.nih.gov/38531011/?_kx=fPH2OXAt7tc3Y6_Quc4e8A.XHRnps" target="_blank" rel="noreferrer noopener">Women Living Better survey titled &#8216;Not Feeling Like Myself (NFLM) in Perimenopause&#8217;</a>&nbsp;to explore what this really means. Here&#8217;s what they found:</p>



<ul class="wp-block-list">
<li><strong>63.3% of women reported &#8220;Not Feeling Like Myself&#8221; more than 50% the time over the past three months.</strong></li>



<li>The main symptoms making us feel unlike ourselves include fatigue, feeling overwhelmed, low moods, anxiety, irritability, and difficulty concentrating.</li>



<li>Our healthcare providers need better education about perimenopause symptoms so they can better support us.</li>
</ul>



<p class="wp-block-paragraph">Clearly NFLM is a real thing that many of us will go through. So let&#8217;s explore it together&#8230;</p>
